Waiver and Release of Liability / 免責事項および責任の放棄

In consideration of being allowed to participate in the Mingoia 3s Ice Hockey Tournament, I hereby acknowledge and agree to the following:

I understand that ice hockey is a physically demanding sport that involves risks of injury, including but not limited to minor injuries, serious injuries, and death.

I voluntarily assume all risks associated with my participation in the Mingoia 3s Ice Hockey Tournament, including but not limited to falls, collisions with other participants, equipment failure, and any other hazards that may arise.

I acknowledge that any guests invited by me are my sole responsibility. I agree to inform all invited guests of the inherent dangers of the rink environment, including but not limited to flying pucks, collisions, and other hazards. I further acknowledge and agree that my invited guests must accept that they cannot hold the rink, John Andrews, David Leon, Andrews LLC, the tournament organizers, sponsors, volunteers, or any other associated parties liable for any injury, damage, or loss that may occur.

I hereby release, waive, discharge, and covenant not to sue John Andrews, David Leon, Andrews LLC, the tournament organizers, sponsors, volunteers, or any other participants in the Mingoia 3s Ice Hockey Tournament for any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my invited guests while participating in or attending the tournament, whether caused by the negligence of the released parties or otherwise.

I understand and agree that this waiver and release of liability is binding upon me, my heirs, executors, administrators, assigns, and any guests I invite, and that it shall be governed by the laws of the state/province/country in which the tournament takes place.

I certify that I am physically fit to participate in the Mingoia 3s Ice Hockey Tournament and that I have not been advised otherwise by a qualified medical professional.

I also grant permission for photographs and videos of me taken during the tournament to be used for promotional purposes by the tournament organizers and sponsors.

If I am signing up for a team, I acknowledge that I am responsible for my team's adherence to these conditions, including any guests associated with my team, and the tournament organizers are not liable for my team's actions.

I have read this waiver and release of liability, fully understand its terms, and voluntarily agree to its provisions by selecting "Accept" below.

私は、ミンゴイア3sアイスホッケートーナメント（以下「本トーナメント」）への参加にあたり、以下の事項に同意します。

アイスホッケーが負傷または死亡の危険を伴う競技であることを理解し、本トーナメントへの参加に伴う一切のリスクを自ら負担します。
私は、招待したゲストに関する一切の責任を負い、当該ゲストに対し本トーナメントに内在する危険性を説明し、主催者その他関係者に対する請求権を放棄させるものとします。
私は、本トーナメントの主催者、スポンサー、ボランティア、関係者（以下「被免責者」）に対し、私またはゲストが被る一切の損害、負傷または死亡について、被免責者の過失の有無を問わず、いかなる請求も行わず、これを放棄し、被免責者を免責します。
本同意は、私の相続人、承継人および関係者にも拘束力を有し、本トーナメント開催地の法令に準拠します。
私は、本トーナメントに参加可能な健康状態にあり、医療上の制限を受けていないことを表明・保証します。
私は、本トーナメント中に撮影された写真および動画が、主催者およびスポンサーにより無償で広報・宣伝目的に使用されることに同意します。
チーム参加の場合、私はチームおよび関係者の本条件遵守について責任を負い、主催者がこれらに関して責任を負わないことを確認します。

以上、本内容を理解の上、自発的に同意します。
